Prof-Care provides a seamless continuation of care from the hospital care to homecare.

From time to time, NHS hospitals discharge patients before they are ready to care for or support themselves at home.

Sometimes they are returned home without appropriate care arrangements in place resulting in poor recovery due to lack of suitable care services in their homes.

We offer immediate care services, for people who are still ill or in the recovery process and need support after being discharged from hospital. 

Support packages

We offer flexible home visits that can be tailored to meet all indivudual needs. Our home care visits range from short, multiple, daily visits to longer and occastional home care visiots.

We will work with you to create a beskoke care plan that can include:

  • 30, 45, or 60 minute home care visits
  • Multiple visits per day
  • Full-day cover
  • Overnight support
  • 24hr live-in care

We also offer a temporary care service  to relieve a family member to take a break or go on holiday according planned arrangements.  We are able to provide services to any clients within Sheffield and the surrounding areas.

We can tailor our services are to meet the immediate individual care and emotional needs of the discharged patient. As standard our carers will help with:

  • Risk assessment and care planning
  • Personal care including bathing/showering and dressing
  • Domestic duties such as general cleaning, laundry, shopping, etc
  • Preparing meals and assisting to eat
  • Social companionship
  • Assist and work with relevant designated health professionals e.g. Social Workers.
